Since, times immemorial art has played a crucial role in raising social issues and even revolutionizing movement against oppression. Theatre, literature, cinema and music are the soft power tools that have been used against imperialism and dictatorship. Similar to the Black Theatre and Black Arts movement in America created the renaissance through artistic activism, IPTA also played a crucial role in becoming the voice of the downtrodden sections of society.

IPTA’S CONNECTION WITH DR HOMI JEHANGIR BHABHA

During the national conference held at the Marwari school of Mumbai on May 25, 1943, IPTA was formed. It came into existence in response to the need for theatre artists to become active volunteers and part of Indian independence movement. The inception of IPTA came from the first Progressive Writer’s Association Conference that was held in 1936. It was followed by the establishment of the Youth Cultural Institute at Calcutta in 1940, and the setting up of the People’s Theatre in Bangalore by Anil De’ Silva in 1941. The name People’s Theatre was suggested by Indian nuclear physicist and the “father of the India nuclear programme” Dr Homi Jehangir Bhabha. Dr Bhabha was inspired by Romain Rolland’s book on the concepts of People’s Theatre. During the initial days of IPTA, Binoy Roy of the Bengal Cultural Squad organised street plays to inform people of the 1942 manmade famine in Bengal. IPTA in association with screenwriter-filmmaker Khwaja Ahmad Abbas (one of the initial members of IPTA) produced Dharti Ke Lal in 1946. The film marked the on-screen debut of acting legend Balraj Sahni. Dharti Ke Lal was based on Bengal famine of 1943. For the unversed, the famine in the Bengal province of British India (present-day Bangladesh, West Bengal, Odisha and eastern India) during World War II, killed millions of people.

BALRAJ SAHNI, DHARTI KE LAL AND THE BENGAL FAMINE

Balraj Sahni began acting with plays of IPTA. Sahni met film director Khwaja Ahmad Abbas as a co-member of the people’s theatre group. Their cinematic collaboration Dharti Ke Lal had a huge impact on cinema, society and culture even after many decades. The New York Times, in its review called it, “…a gritty realistic drama.” The neo-realist theme not only inspired future scriptwriters and filmmakers, but also left a long-lasting impact on India’s left-wing intellectuals. The movie also introduced versatile actress Zohra Sehgal to Indian cinema. Dharti Ke Lal also had a cross-border impact due to the socialist-communist influence on the narrative. The rebel and anarchy of the left-wing was considered the only way forward during that time which could lead to liberation and empowerment. Dharti Ke Lal became the first Indian film which received widespread distribution in USSR (Union of Soviet Socialist Republic). This paved way for the Soviet Union becoming a major overseas market for Indian films.

Legens like Prithviraj Kapoor, Ritwik Ghatak, Salil Chowdhury and Pandit Ravi Shankar were also among the initial members of IPTA.

Popular Weight-Loss Drugs Like Wegovy May Raise Risk Of Complications Under Anesthesia https://www.india.com/lifestyle/popular-weight-loss-drugs-like-wegovy-may-raise-risk-of-complications-under-anesthesia-6225136/ Sun, 13 Aug 2023 17:55:58 +0000 https://www.india.com/?p=6225136 Weight-Loss Drugs: Patients who take blockbuster drugs like Wegovy or Ozempic for weight loss may face life-threatening complications if they need surgery or other procedures that require empty stomachs for anesthesia. This summer’s guidance to halt the medication for up to a week may not go far enough, either.

Some anesthesiologists in the U.S. and Canada say they’ve seen growing numbers of patients on the weight-loss drugs who inhaled food and liquid into their lungs while sedated because their stomachs were still full — even after following standard instructions to stop eating for six to eight hours in advance.

The drugs can slow digestion so much that it puts patients at increased risk for the problem, called pulmonary aspiration, which can cause dangerous lung damage, infections and even death, said Dr. Ion Hobai, an anesthesiologist at Massachusetts General Hospital in Boston.

“This is such a serious sort of potential complication that everybody who takes this drug should know about it,” said Hobai, who was among the first to flag the issue.

Nearly 6 million prescriptions for the class of drugs that include Wegovy and Ozempic were written between January and May in the U.S. for people who don’t have diabetes, according to Komodo Health, a health care technology company. The drugs induce weight loss by mimicking the actions of hormones, found primarily in the gut, that kick in after people eat. They also target signals between the gut and the brain that control appetite and feelings of fullness, and by slowing how fast the stomach empties.

In June, the American Society of Anesthesiologists issued guidance advising patients to skip daily weight-loss medications on the day of surgery and hold off on weekly injections for a week before any sedation procedures. Dr. Michael Champeau, the group’s president, said the action was based on anecdotal reports of problems — including aspiration — from around the country.

It’s not clear how many patients taking the anti-obesity drugs may be affected by the issue. But because the consequences can be so dire, Hobai and a group of colleagues decided to speak out. Writing in the Canadian Journal of Anesthesia, they called for the drug to be stopped for even longer — about three weeks before sedation.

That accounts for how long semaglutide, the active medication in Wegovy, remains in the body, said Dr. Philip Jones, a Mayo Clinic anesthesiologist who is also deputy editor-in-chief of the journal.

“When 90% of it is gone, which is after three weeks, hopefully everything should go back to normal,” Jones said.

Champeau and Jones both acknowledged there’s not enough evidence to say for certain how long semaglutide should be held to make anesthesia safe. Many patients won’t see providers far enough in advance to stop the drug three weeks before procedures, Champeau noted.

Aspiration occurs in one of every 2,000 to 3,000 operations that require sedation, and almost half of patients who aspirate during surgery develop a related lung injury. But case reports show recent patients on semaglutide had problems even when they stopped food as long as 20 hours before their procedures.

“There’s nothing that says if you fast twice as long, it will be OK,” Champeau said.

Among the several reports detailing potentially serious problems was one of Hobai’s patients, a 42-year-old man in Boston who recently began taking Wegovy, had to be intubated and suffered respiratory failure that put him in intensive care. He aspirated food that remained in his stomach despite fasting for 18 hours.

In Chapel Hill, North Carolina, a 31-year-old-woman on a low dose of Ozempic had fasted for 10 hours before a routine endoscopy prior to bariatric surgery last fall. The procedure had to be stopped because solid food remained in her stomach and she was at high risk for aspiration, the report said.

Since then, doctors have seen dozens of similar cases as use of the weight-loss medication has grown, said Dr. Elisa Lund, an anesthesiologist at the University of North Carolina at Chapel Hill School of Medicine. “It has exponentially increased,” she said.

Hobai is completing a retrospective study of almost 200 patients taking semaglutide. While it’ll be published later this year, the work so far appears to confirm a small study from Brazil, he said. In that study, about a quarter of patients taking semaglutide had residual food in their stomachs during procedures requiring sedation — even after stopping the drug for 10 days.

The American Society of Anesthesiologists advises doctors who are in doubt to treat patients who haven’t paused the drug as if they have full stomachs, which can mean using different types of sedation protocols or delaying procedures, if possible. Jones added that research is urgently needed to update guidelines for doctors and patients.

Novo Nordisk, which makes Ozempic, Wegovy and similar drugs, said the firm’s clinical trial and post-marketing safety data did not show that the medications led to aspiration. But the drugmaker noted that the medications are known to cause delayed emptying of the stomach and that the labels warn of possible gastrointestinal side effects.

Stopping the medications for three weeks can cause problems, too. Patients with diabetes will need another way to control their blood sugar and those seeking to lose weight may regain some, Hobai said.

Hobai suggests that people using Wegovy and similar drugs tell their doctors before sedation and discuss the risks and benefits.

“If you’re taking this drug and you need an operation, you will need to have some extra precautions,” he said.

PREPARATION METHOD

  • Boil coconut Milk: Now place a saucepan on flame and pour coconut milk dissolved in coconut water and let it boil, add sugar, kafir lime and wait until it releases flavor, then remove from flame.
    Dissolve agar agar: Place another pot or saucepan on flame and pour about 150 ml of water and add the agar agar into it. Switch flame to high mode and let it boil well, wait until dissolve. Wait patiently, it will dissolve after about 10 to 15 minutes of time.
  • Now make the kiwi puree and put in a boiling water as per consistency. once it starts boiling add agar agar and let it dissolve. Add litchi juice to adjust the sweetness, once the mixture is completely dissolved remove from flame and put in a glass jar for the layer to set.
  • Now pour one third of agar agar into hot coconut milk and stir well. Add little salt also. Now cut the oranges and take out the chunks. add orange juice for boiling, add agar agar and let it dissolve, once done add orange segments into it. Layer the glass jar with second layer with coconut milk and keep in the fridge for setting the layer. once done take it out and put the third layer with orange juice.
  • Refrigerate or let it set in room temperature, but refrigeration will help quicken the process. Use a ladle to pour the mixture and be very gentle while pouring, see that there are no air bubbles while you pour. Try pouring equal levels to make sure it looks good. Refrigerate or set aside until it sets well.

Adjusting To Survive

I was in grade five and for the first time appeared for the handwriting contest held for the entire class. No shame to acknowledge that not only did I perform miserably but was also chided for having horrible handwriting. Oh yeah, every move for the next letter had smudged the ink* making the handwriting contest more like a contest for abstract art. All this happened because I wrote the test with my left hand, being born a left-hander. Even years after this incident, I live with the pain and struggle of trying to “survive in the right-hander’s world”.

*I belong to the old school

As a lefty who has been forced to use right-handed desks, it can only be imagined the extra effort and strength we lefties put in as we jot down notes while trying to maintain the balance to stay seated and decently at the desk.

We manage to adjust to the right-handed desk but there is another ordeal waiting for us. Those who used ink pens years back (ink pens are almost extinct) will agree that we had an uncanny knack for attracting the colourful liquid. Maybe it was a disgruntled lefty who invented the gel pen.

Not Many For Armed Forces, Killing The Hobby

Somehow, I spent a good number of sessions at the combat ranch and scored well as I practiced several firearms, small to big ones, and all of them are still designed for the right-handers as far as I am aware. And fewer people who are left-handed make it to the armed forces or other forces for the same reason.

Then back at home, it is the pair of scissors and peelers that are not friendly to left-handed people. Again, they are designed for the benefit of the righties.

I simply love music and always wanted to, and still want to be an ace guitarist like my heroes Steve Stevens, Jimmy Page, and David Gilmour. I did not and do not have the patience to do a Jimi Hendrix or Sir Paul McCartney kind of modification to the six strings. But I had to settle for being the lead vocalist because most musical instruments, guitars too, are designed for right-handers.
